HIPAA Notice of Privacy Practices
Midwest Brain and Spine holds all information pertaining to the care and treatment of our patients in the strictest confidence. All information in the patient's medical record is maintained with the utmost care and respect to preserve privacy and confidentiality. Midwest Brain and Spine fully complies with the Federal Governments mandated HIPAA requirements for patient confidentiality and privacy of healthcare information. NOTICE OF HIPAA PRIVACY PRACTICE that outlines the circumstance for which we can disclose protected health information without authorization. Only a patient can provide the authorization to release records necessary for Midwest Brain and Spine to disclose protected health information for instance not related to your ongoing treatment and/or payment of claims
Office Hours and Appointments
Office Hours - Our office and telephones are open from 8:00 am to 4:30 p.m., Monday through Friday.
Scheduling and Appointment- When scheduling an appointment, please describe your needs to our staff so that an appropriate length of time and day can be reserved for you. We also make every effort to see patients at scheduled appointment times, as we realize that your time is valuable. However, sometimes we do fall behind schedule or are called out of the office to handle emergency cases. Our staff will attempt to notify you by telephone if there is an emergency situation and your appointment time will be schedule as soon as possible.
Cancelling an appointment - If you cannot keep your appointment, please contact our office as soon as you know you will not be able to keep your appointment.
Confidentiality
Your medical information is strictly confidential. We will not release it to anyone without your written consent. A family member may however, accompany you to your appointments if you wish. If you would like a copy of your records sent to another physician we will require a written authorization from you.
Prescriptions and Refills
Try to request new prescriptions or refills at appointments, when possible. Refills will only be made during normal business hours, as we must review your medical chart. Please have your pharmacy's phone number available. We discourage prescribing new medications over the telephone, as an accurate diagnosis is usually possible only with an office visit. Prescriptions will only be refilled for medication prescribed by Dr. Maioriello.
Fees and Payment
Co payments are required prior to your visit or we will be happy to reschedule you appointment when co payment can be made. For services not covered by insurance, we accept cash or personal checks. Return checks will be assessed a $25.00 return fee. Our billing department will accept VISA, MasterCard and will be happy to work with you to pay any balances due. As a new patient you are required to sign a financial agreement, stating that you are ultimately responsible for any unpaid balance our your account. A $20.00 fee will be charged for each individual medical claim or disability form requiring completion by our office.
Health Insurance with "Out of Network" Benefits
For insurance plans with which we do not participate, it is common that out of network benefits apply. Please check with your carrier to determine your network and out of network benefits. We will submit that insurance claim for you and you will be required to pay the out of network fees.
Emergency Care
Please limit after-hour calls to urgent and emergency needs. If you have a post surgical situation, please call our office and follow the prompts according to you situation. If you feel you have an emergency, please go to your nearest emergency room and the on call physician will be able to assist you.
